Job summary

Ontario HealthCare Center is seeking a Physical Therapist to evaluate, plan, and implement individualized therapy programs in a skilled nursing setting. The role emphasizes compassionate, respectful care, thorough documentation, and collaboration with physicians, nurses, and therapists.

The position requires a DPT or master’s in PT with valid state licensure. Experience in SNF settings is preferred, with new graduates encouraged to apply.

Qualifications

• Doctorate or master’s degree in physical therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Previous experience in a skilled nursing facility or similar setting preferred.
• New graduates with strong clinical skills are encouraged to apply.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of regulatory requirements and best practices in physical therapy.

Responsibilities

• Conduct thorough evaluations of residents' physical abilities and limitations.
• Develop individualized treatment plans based on assessment findings.
• Implement physical therapy interventions to improve strength, mobility, balance, and coordination.
• Utilize various therapeutic techniques and equipment to achieve treatment goals.
• Monitor and document residents' progress and adjust treatment plans as necessary.
• Work closely with other members of the healthcare team, including physicians, nurses, and occupational therapists.
• Communicate effectively with residents and their families regarding treatment plans and progress.
• Participate in interdisciplinary team meetings and care planning sessions.
• Maintain accurate and timely documentation of all patient interactions and treatments.
• Ensure compliance with all federal, state, and local regulations and facility policies.
• Participate in quality assurance and performance improvement activities.
• Provide education and training to residents and their families on exercises and activities to be performed at home.
• Offer support and encouragement to residents to help them achieve their rehabilitation goals.
