Qualifications

Pay Range: $21.00 - $26.00/hour

Job Summary

The Assistant is responsible for assisting professional staff by carrying out various resident care activities and related non‑professional services necessary for caring for the personal needs and comfort of the residents. Responsibilities include the delivery of resident care such as baths, personal grooming, and feeding directed by the supervisor(s). The role maintains departmental policies and procedures, quality assurance programs, safety, environmental and infection control standards, and incorporates the facility’s philosophy and mission with total regard for individual human dignity.

Position Accountability and Performance Criteria

1. Personal care assistance: administers complete or partial resident baths, oral care, dentures, Foley care, hair/nail care, shaving, bed making and cleaning of utensils. Assists with toileting, records outputs accurately, empties drainage devices, and completes documentation in the BM Book and toileting schedule. Follows facility protocol for admission documentation, follows nursing process, adheres to HIPAA competency protecting PHI. Reads and follows resident care plans, maintains harmonious relationships with unit and coworkers.

2. Meal preparation: prepares residents for meals, serves and collects trays, assists with setting up and feeding residents, serves fresh water and nourishments when indicated. Completes meal assessment sheets and records oral intakes accurately. Uses proper lifting and body mechanics to prevent injury; demonstrates cooperation and professionalism, organizing work to meet required time frame.

3. Resident transfer and restraint: helps residents with transferring and lifting, raising residents in beds, turning residents. May apply restraints as directed. Maintains confidentiality of resident, facility and physician information. Uses appropriate transfer devices and applies restraints following policy. Reports problem areas to supervisor and seeks help when needed.

4. Vital signs and specimens: obtains temperature, pulse, respiration, weight, intake/output, and urine samples as warranted. Records findings, assists with specimen collection (urinalysis). Recognizes and reports changes in resident condition, asks clarifying questions, and reports results to achieve best care.

5. Resident comfort and communication: checks on residents to ensure comfort, dignity, and privacy; answers call lights and inquiries promptly; reports abnormal findings. Communicates clearly and tactfully with residents and their families. Collaborates with personnel to achieve department objectives and maintain good employee relations.

6. Unit operations and room readiness: performs related duties as assigned, prepares resident room for admission, handles orientation, provides admission pack supplies, assists with dressing. Supports nurse with duties such as removing/ replacing TEDS, reapplication of oxygen, specialty mattresses, turning immobile residents. During discharge assists with dressing and packing, cleans room supplies/linen before terminal cleaning, assists with post‑mortem care. Communicates effectively, maintains resident optimal functioning and rehabilitation, reports changes to charge nurse.

7. Professional image and in‑services: attends all required in‑services and meetings, presents poise and authority, wears appropriate clothing and ID badge, follows infection control competency and hand‑washing procedures, participates in drills and plans.

8. Unit cleaning and efficiency: cleans assigned unit areas, includes IV poles, ice bucket/ladle, wheelchairs/carts; oversees containment of costs by conserving supplies. Organizes work for timely task completion, responds to workload changes, cleans utility rooms/pantry, delivers supplies, maintains neat and clutter‑free resident rooms, labels items.

9. Other duties: performs additional duties as assigned or requested.
