Job summary

Love and Loyalty Home Care is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing in patients' homes in Virginia. The role emphasizes hands-on care, accurate assessment, and collaboration with a RN-led team.

Qualifications

Responsibilities

Provide skilled nursing services within your scope of practice and plan of care.
Perform nursing observations and assessments per protocols.
Monitor patients and recognize changes in condition.
Communicate clinical concerns to RN/CNO.
Perform authorized nursing treatments per physician orders.
Support medication management and patient education.
Maintain accurate clinical documentation.
Communicate professionally with patients, families, physicians, and team.
Assist with clinical intake and care coordination.
Collaborate with RN and leadership for consistent care.
