Presbyterian SeniorCare Network is seeking a Service Coordinator/Social Worker for Etna Commons/Plum Creek in Pittsburgh, PA. This full-time, hourly role offers daylight hours with starting pay ranging from $21.00 to $26.00 per hour depending on experience.

The Service Coordinator/Social Worker is responsible for establishing ties to service providers and agencies in the community, referring residents to such providers and monitoring the provision of these services. The Service Coordinator is responsible for the monitoring of services that address the social determinants of health and bridge gaps between communities, individuals and providers.
