Far-Northern-Regional-Center in Chico, CA is seeking a Service Coordinator in the Case Management department to provide dedicated case management services to individuals with developmental disabilities. The role develops and monitors Individual Program Plans and coordinates services with partner agencies.

Responsibilities include home visits, consultations with clinical staff, client advocacy, completing reports, maintaining case records, and supporting clients and families.

• Requires equivalent to a B.A. Degree in field related to Social Welfare or one year of experience in a like position.

SALARY & BENEFITS

$29.14 - $37.88 per hour. Complete package of fringe benefits including Vacation and Sick Leave, Health Insurance, 17 paid Holidays, and CalPERS Retirement. Travel allowance for field work.
