Position Overview

Service Coordinator – Under the general direction of a Case Management Supervisor, responsible for the full range of case management duties including intensive case management and high levels of expertise. Implements client's service plans, assists in confirming Lanterman eligibility for young children, manages delivery of services, ensures Individual Program Plan objectives are pursued, assists in resource utilization, monitors vendor services, evaluates effectiveness of plans, and conducts annual reassessments with interdisciplinary team. May conduct client assessments and occasionally act in supervisory or lead capacity.

Key Responsibilities
  • Manage delivery of services to regional center clients with intensive physical or emotional needs.
  • Ensure the client’s Individual Program Plan and objectives are pursued.
  • Assist clients in using public and private resources before accessing regional center funds.
  • Monitor vendor services and verify services are delivered per purchase authorizations.
  • Evaluate effectiveness of Individual Program Plans.
  • Ensure appropriate community resources are available to the client.
  • Conduct annual reassessments of clients’ developmental status with interdisciplinary team.
  • May conduct client assessments on an occasional basis.
  • Initially or on an ongoing basis support young children (3-6) with Lanterman eligibility determinations.
  • Occasionally act in supervisory or lead capacity (e.g., Officer of the Day or Acting Supervisor).
Minimum Education & Experience
  • Master’s degree in a field related to developmental disabilities, OR
  • Bachelor’s degree in a related field (social work, psychology, counseling, special education, nursing) and three years’ employment in a related field. Minimum education requirement may not be waived.
  • Minimum qualifications may be waived depending on agency needs for specialized skills (such as language abilities).
Special Requirements
  • Fluency in a language in addition to English is desirable and may be required.
  • Possession of a valid California driver’s license and minimum legal automobile liability insurance; ability to provide personal automobile transportation during working hours and after‑hours emergencies or access to reliable transportation.

Bi‑lingual Language Differential
