Job summary

Qualifications

  • Requires a Bachelor's or advanced degree from an accredited college/university.
  • Preferred fields: social, behavioral, human services (Bachelor’s/Master’s).
  • One year of experience with Intellectual Developmental Disability population (required).

Responsibilities

  • Meets with developmental disability customers at their homes to assess needs and determine required services.
  • Explains services to customers and presents recommendations based on needs.
  • Acts as primary customer contact and ensures customer satisfaction is achieved.
  • Keeps records in our computer system and coordinates services with providers.

Alongside clinical care, researchers and teachers from Metrocare’s Altshuler Center for Education & Research are advancing mental health beyond Dallas County while providing critical workforce to the state.Job Description:GENERAL DESCRIPTION:The mission of Metrocare Services is to serve our neighbors with developmental or mental health challenges by helping them find lives that are meaningful and satisfying. We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The Service Coordinator advocates and represents the desires of the individual in service delivery. The Service Coordinator also provides assistance in problem resolution between the provider and the consumer. The Service Coordinator will also be monitoring to determine if specialized services are provided and that the services are helping the person to achieve their desired outcomes.ESSENTIAL DUTIES AND RESPONSIBILITIES:The essential functions listed here are representative of those that must be met to successfully perform the job.Meets with developmental disability customers at their homes, listening carefully to ascertain their needs, and helping them determine what services they require.Makes recommendations for, and explaining services to customers, based on their needs.Takes customers initial service requests.Enters information into our computer system.Helps customers access services to help them achieve the quality of life they desireMeets with customers monthly or quarterly to ensure they are getting needed services, to evaluate the effectiveness of the services in meeting needs, and to determine new problems and new interventions.Meets with developmental disability services providers to learn their services, to arrange services for customers, to advocate for our customers, and to make sure they are taking care of our customers.Assists when a crisis arises, be available to help link and assist customers to secure services that will enable them to prevent future crisesActs as primary customer contact, ensuring that satisfaction is the end result.Performs other duties as assigned.COMPETENCIES:The competencies listed here are representative of those that must be met to successfully perform the essential functions of this job.Conducts job responsibilities in accordance with the ethical standards of conduct, state contract, appropriate professional standards and applicable state/federal lawsAnalytical skills, professional acumen, business ethics, thorough understanding of continuous improvement processes, problem solving, respect for confidentiality, and excellent communication skills.Self-motivating, able to work independentlyBasic computer skills, Windows 95 and Microsoft Word. Prefer experience with AskesisStrong organizational and time management skillsStrong oral and written communication skillsQUALIFICATIONSEDUCATION AND EXPERIENCE:Required: Bachelor's or advanced degree from an accredited college/university.Preferred: Bachelor’s or advanced degree from an accredited college/university in the field of: social, behavioral, human services. Required: One year of experience working with Intellectual Developmental Disability Population.ORMaster's degree in preferred list of fields with 0 years of experienceMATHEMATICAL SKILLS:Basic math skills required.REASONING ABILITY:Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.COMPUTER SKILLS:Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).Ability to utilize Internet for resources.CERTIFICATIONS, LICENSES, REGISTRATIONS:Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.Liability insurance required if employee will operate personal vehicle on Center property or for Center business. Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business.
