Job summary

Trivium Life Services is seeking a Service Coordinator to support individuals receiving services. You will develop person-centered plans, implement behavioral plans, and ensure coordinated services with families, providers, and staff, while maintaining confidential records.

Qualifications

  • High School Diploma or equivalent required.
  • Two years' experience serving people with disabilities in a service delivery role.
  • Demonstrated strong organizational and analytical skills.
  • Must obtain Medication Manager certification within six months.
  • Proficient in myEvolv or similar EHR.

Responsibilities

  • Develop, monitor, and support an Individual Support Plan with the client.
  • Write, coordinate, and implement behavioral plans.
  • Advocate for clients and coordinate needs with stakeholders.
  • Coordinate intake, discharge, and client authorization; manage client data in myEvolv.
  • Ensure adherence to medication management plans for clients.
  • Maintain HIPAA compliance and safeguard sensitive information.
  • Coordinate services to meet regulatory standards (CARF, HCBS, DoIA).
  • Update client data systems for goals and compliance.
  • Communicate with direct support staff to ensure quality care.

Description

Position

The Service Coordinator is responsible for providing support to people receiving services from Trivium Life Services. This role is responsible for ensuring each person served is provided the opportunity to enhance their life through the process of a person-centered plan. The Service Coordinator will report directly to the Service Coordinator Manager or the Market Director.

Essential Duties

This position includes, but is not limited to, the following essential functions:

  • Develop, monitor, and support an Individual Support Plan in collaboration with person served, to achieve goals.
  • Write, coordinate, & ensure the implementation of behavioral plans.
  • Effectively advocate for persons served both internally and externally and coordinate individual needs of persons served and maintain quality relationships and open communication channels with all stakeholders (Person served residential providers, employers, social workers, case managers, funders, families, guardians, staff, etc.)
  • Perform persons served intake, discharge, termination, and follow-up tasks, working with Finance to secure and process client authorization for both new and existing clients. Maintain accurate and timely client information in myEvolv (EHR); be a super-user and champion the tool
  • Ensure adherence to persons' served medication management plans.
  • Abide by all HIPAA regulations and employ safeguards as directed by HIPAA laws and the Trivium Compliance Officer.
  • Coordinate services to meet CARF standards, Home and Community Based Services, Department of Inspections and Appeals, and other regulatory agencies.
  • Update client data systems on a regular basis to ensure accuracy for goal planning and compliance.
  • Maintain open communication channels with direct support staff to provide the supports needed and ensure consistency and quality of client care. Work collaboratively with the Team Coordinator(s) as it relates to client services.
  • Ensure and assist in coordination with Residential Supervisors working with social security to provide payee services as needed, to aid persons served in the use of work incentives, provide wage reporting, and benefit counseling, and supervise the management of persons served personal finances, as well as low-income assistance. (as needed and desired).
  • Participate in meetings for coordinating with the person served and their interdisciplinary team.
  • Update market leaders as scheduled and as needed to keep them aware of persons served; also, the Service Coordinator Manager where applicable.
  • Facilitate family member support in achieving and maintaining the assigned individual's goals for all Trivium programs in a holistic manner and in line with an overall enhanced client experience.
  • Ensure service files of persons served are organized and kept confidential.
  • Document client interactions and support interventions in the form of progress notes, incident reports, GERs, communication log, and any other as needed and instructed.
  • Participate in the outcome measurement system as an integral person in the workflow and reporting.
  • Assist with supervision of persons served as needed.
  • Other duties as assigned by supervisory personnel.

Physical Demands

These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the Service Coordinator position. While performing the responsibilities of the Service Coordinator, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl. Vision abilities required by this job include close and distant vision.

Working Conditions

Working conditions are normal for an office environment. The noise level in the work environment is usually quiet to moderate. Work will require weekend and/or evening work as needed. Occasional conditions will vary as it will be necessary to be in various locations, homes of persons served or their families and funding agencies. The employee must be able to travel between locations, and occasionally attend seminars which may require an overnight stay.

Requirements

The Service Coordinator will have the following experience and attributes:

  • High School Diploma or equivalent.
  • Two years' experience in working with people with disabilities in a service delivery related field.
  • Demonstrated experience in strong organizational and analytical skills; and demonstrate the ability to organize, manage and execute responsibilities under limited time frames within a fast paced and changing environment.
  • Must obtain Medication Manager certification within six months of employment.
  • Must be proficient in myEvolv or utilized EHR within six months.

The following are preferred but not required:

  • Associate degree from an accredited college or demonstration of a minimum of 60 college credit hours.
