General Summary

The Sepsis Coordinator collaborates with the healthcare team through daily clinical rounds to plan, implement, monitor, and evaluate care for sepsis patients. The role focuses on health promotion, disease prevention, and education for patients, families, and staff.

Responsibilities
  • Facilitate the implementation and evaluation of the sepsis program, including all systems necessary for a multidisciplinary approach across the continuum of care.
  • Conduct regular rounds on sepsis patients to assess treatment plans, nursing interventions, physician documentation, and compliance with the sepsis bundle, ensuring timely, guideline‑concordant care.
  • Provide education to patients, families, and staff and evaluate their understanding.
  • Document care delivery promptly and professionally, collecting, analyzing, and submitting CMS sepsis core measure data to designated regulatory agencies.
  • Analyze data to identify opportunities to improve early identification and treatment of severe sepsis and septic shock patients.
  • Facilitate close multidisciplinary partnership between the ED, ICU, MS, and other departments.
  • Educate staff on sepsis best‑practice treatment and inform physicians on documentation guidelines.
  • Provide real‑time, detailed feedback to clinical providers and departments.
  • Coordinate and facilitate the Sepsis Committee and work groups with physicians and leadership.
  • Collaborate with the Quality Performance Improvement Department, Nursing, Education, Physician Leadership, and Marketing to advance program projects and sepsis education.
  • Continuously improve outcome measures for severe sepsis and septic shock patients, including:
    • Improvement in compliance with the Surviving Sepsis Campaign Resuscitation and Management Bundle
    • Reduction in length of stay
    • Reduction in mortality rates
Education
  • Provide formal and informal education to medical and clinical staff.
  • Collaborate with nursing management, the education department, and Medical Staff to develop and implement an education model based on current resources.
  • Serve as a resource for staff to support program goals.
  • Support EMR development, including RN screening tools, MD order sets, and documentation tools.

Job Specifications
Knowledge
  • Graduate of an accredited school of nursing.
