EmpowerMe Wellness is seeking therapists to provide physical, occupational, or speech therapy to seniors within our EmpowerMe 360 program. You will assess residents, design personalized care plans, and deliver evidence-based interventions as part of a collaborative care team.

Qualifications

• Current professional license in therapy discipline (PT, PTA, OT, COTA, or SLP).
• Strong clinical and interpersonal skills.
• Ability to work collaboratively with a care team.
• Passion for empowering seniors and improving independence.

Responsibilities

• Provide therapy and wellness services for older adults (physical, occupational, or speech).
• Conduct assessments, create care plans, and deliver evidence-based interventions.
• Support care coordination with wellness check-ins and telehealth support.
• Maintain professional and ethical standards per company initiatives.
• Contribute to program development and quality improvement.
• Help residents maximize independence, confidence, and satisfaction.
