Job summary

EmpowerMe Wellness is seeking licensed therapy professionals to join our EmpowerMe 360 program, delivering physical, occupational, or speech therapy to seniors in a collaborative care setting.

You will assess residents, design personalized plans, and deliver evidence-based interventions while coordinating with wellness checks and telehealth. Growth opportunities, flexible scheduling, and a supportive culture are offered.

Qualifications

• Current professional license in your therapy discipline (PT, PTA, OT, COTA or SLP).
• Passion for empowering seniors to live independently and safely.
• Strong clinical skills and ability to work within a collaborative team.
• Commitment to teamwork, community engagement and patient-centered care.

Responsibilities

• Provide therapy and wellness services (physical, occupational, or speech) for older adults.
• Assess residents, create personalized care plans, and deliver evidence-based interventions.
• Support care coordination with wellness check-ins and telehealth support.
• Maintain professional and ethical standards per company initiatives.
• Contribute to program development and continuous quality improvement.
• Help residents maximize independence, confidence, and satisfaction.
