EmpowerMe Wellness in Illinois is seeking a licensed therapy professional to deliver care to older adults through the EmpowerMe 360 program. You’ll provide therapy and wellness services, perform assessments, and develop personalized care plans within a collaborative team.

• Current professional license in PT, PTA, OT, COTA, or SLP.

Responsibilities

• Provide therapy and wellness services for older adults
