Responsibilities

• Assess patient health status and develop nursing plans of care.
• Complete POE and address care gaps during patient encounters.
• Accurately document patient data and responses to treatment.
• Assist with procedures and monitor patient responses.
• Engage patients, families, and caregivers in care coordination.
• Lead the nursing response to medical emergencies.
• Monitor electronic messaging related to patient or provider requests.
• Initiate and monitor IV therapy and medication administration.
• Assess health education needs and provide counseling.
• Conduct follow-up calls to patients requiring care.
