Job summary

The Senior Physical Therapist, Occupational Therapist, and Speech and Language Pathologist at Jefferson Health are responsible for evaluation, treatment and discharge planning of all patients referred for their services in various care settings. Qualifications

• Master’s Degree in Speech-Language Pathology required.
• 5 years of current clinical practice.
• CCC-SLP - Certificate of Clinical Competence in Speech Language Pathology.
• BLS - Basic Life Support.
• AHA - American Heart Association.

Responsibilities

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Provides therapy evaluations, treatment planning, treatment interventions and discharge planning for a full patient caseload in acute care, skilled facility, acute rehab, outpatient settings as appropriate.
• Plans, organizes and delivers treatment programs that help clients/patients recover and rehabilitate.
• Records and documents patients' health progress information, daily treatments, progress notes, evaluations and discharge summaries.
• Supports supervisor/manager with ongoing clinical and departmental operations.
