Job Title: Senior Specialty Practice Pharmacist – Palliative

Department: James | Pharmacy Administration

Scope of Position

In cooperation with other pharmacy staff, acts as a resource and provides answers to drug information questions for patients, care givers, health care professional students, and medical center staff.

Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions.

Participates in appropriate hospital and/or departmental committees relevant to practice area.

As a senior position, this individual represents the palliative pharmacy team in meetings, oversees scheduling and resource allocation among the team, owns onboarding of new employees, and assists with leading quality improvement activities within the palliative pharmacy practice.

Position Summary

The Ohio State University Palliative Medicine Team is a multi-disciplinary team. As a member of the palliative team, the pharmacist is a valued contributor and is involved in the direct care of patients on a daily basis. Through a robust collaborative practice agreement, the Palliative Care Specialty Practice Pharmacist is expected to initiate, modify, or discontinue medications to optimize symptomatic control. They provide daily comprehensive prospective profile review for all scheduled patients, including assessing disease states, drug-drug interactions, duplication of therapy, drug-allergy interactions, laboratory values, and urine drug screens.

This position requires the pharmacist to conduct daily patient symptom assessment and drug therapy management in order to best serve patients with palliative needs, as well as patients with complicating medical histories, including substance use disorder. The pharmacist may work in both inpatient and outpatient settings focused on delivering high quality palliative care. Collaborate with other providers in opioid stewardship to demonstrate less opioid exposure and safer opioid prescribing within the medical center.

This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas.

Duties include but are not limited to:
  • medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; and coordination of patients' pharmaceutical needs with providers and nursing staff.
  • Patient population includes adult and geriatric patients; must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Collaborate with other providers in opioid stewardship to demonstrate less opioid exposure and safer opioid prescribing within the medical center.
  • Coordinate all aspects of patients' pharmaceutical needs in the patient care areas.
  • Supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (providers, nurses, and pharmacy staff).
Minimum Qualifications
  • Bachelor's Degree in Pharmacy.
  • Registered Pharmacist.
  • 1 year of relevant experience required.
  • 2-4 years of relevant experience preferred.
