Qualifications

Responsibilities

• Complete psychosocial assessments to identify emotional, social, environmental, and financial factors affecting patient care.
• Provide direct patient care including crisis intervention, counseling, education, advocacy, referral management, and mediation.
• Screen for abuse, neglect, IPV, exploitation, and sexual assault; coordinate with agencies.
• Collaborate with interdisciplinary teams to develop and revise care plans.
• Monitor progress and document outcomes; maintain confidentiality and documentation standards.
• Develop programs addressing psychosocial, educational, financial, and healthcare needs.
