Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Licensed or certified by a state at the advanced practice level to independently practice social work.

• Provide individual, group, and family counseling with diverse patient populations.
• Assess psychosocial functioning and patient/family needs; develop treatment plans and goals.
• Advocate with community services and coordinate multidisciplinary care.
• Ensure privacy and confidentiality; document in the medical record per policies.
