Job summary

Atlantic Health System is hiring a Complex Care Coordinator (RN) for Morristown, NJ. This full-time role requires coordinating care with a multidisciplinary team, promoting adherence to the plan of care, and communicating patient findings to providers and care partners across settings.

Required: BSN and NJ RN license, with 5 years of clinical RN experience. Preferred: PA/NY licenses, CCM certification, and community health experience.

Qualifications

• Graduate of an accredited Nursing Program; BSN required.
• Current NJ Registered Professional Nursing license required.
• Current PA and NY Nursing license a plus.
• Minimum 5 years of clinical experience as a Registered Professional Nurse.

Responsibilities

• Coordinate care with the patient’s primary provider and outreach as applicable.
• Identify opportunities for health promotion and disease prevention.
• Collaborate with the multidisciplinary team to develop strategies addressing patient needs.
• Maintain patient confidentiality within the Care Coordination Program.
• Participate in payer webinars and use payer reports to guide outreach and education.
• Collaborate with ACO leadership on additional tasks as requested.
