Job summary

CommuniCare Rehab is seeking a Physical Therapist (PT) for a PRN opening at Burlington Healthcare Center. The role involves evaluating residents, developing and implementing treatment plans, and education for the rehab team to ensure coordinated care.

The candidate will participate in interdisciplinary meetings, provide supervision for PTAs, and may work at multiple centers to meet diverse patient needs. This is a flexible, in-house team position within a growing family of care facilities.

Qualifications

• Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
