Hiring range: $74,214 to $103,896 annually ($35.68 to $49.95 per hour). Starting rate will depend on experience, education, and qualifications. Advancement opportunities exist for individuals motivated to move up within Anoka County.

Position Description

In Public Health and Environmental Services (PHES), commitment to the community shines. Every day our Health Promotion and Planning team works with community partners to create conditions for people to live healthy lives.

The Health Equity Senior Program Planner at Anoka County Public Health and Environmental Services (PHES) aims to enhance the department's capacity to meet foundational capabilities and effectively serve diverse communities. Key responsibilities include developing partnerships, facilitating community visioning, and supporting health programs. The planner also coordinates volunteer programs and community groups and participates in planning committees. Additionally, they research strategies for program improvement, identify service gaps, and recommend best practices for Anoka County. The role includes providing leadership, coaching, and mentoring to the team.

Pay & Benefits
  • Anoka County Salary Schedule Grade 34: $74,214 to $103,896 annually ($35.68 to $49.95 per hour).

Work Location

This position will work at the Anoka County Government Center located at 2100 3rd Avenue, Suite 600, Anoka, MN 55303.

This position may be eligible for flexible work arrangements, including hybrid work, with some days working remotely and some days working in the office.

Expected work hours are Monday - Friday 8:00 a.m. - 4:30 p.m.

Job Duties and Responsibilities

  • Develop and implement health equity plans, strategies, and initiatives to address identified disparities.
  • Integrate health equity principles into all public health planning, policies, and programs.
  • Engage with diverse community members, especially those from marginalized or underserved populations, to understand their health needs and priorities.
  • Build and maintain relationships with community-based organizations, advocacy groups, and other stakeholders to collaborate on health equity initiatives.
  • Facilitate community forums, focus groups, and other participatory processes to gather input and foster community involvement.
  • Collaborate with other government departments and agencies to ensure a coordinated approach to health equity across sectors.
  • Develop communication strategies to raise awareness about health equity issues and the department's initiatives.
  • Serve as a spokesperson on health equity issues, representing the department in public forums, meetings, and media engagements.
  • Work closely with other planners, public health officials, and community partners to ensure health equity is integrated across all public health efforts.
