Job summary

Jefferson Health Plans is seeking a Senior Physical Therapist responsible for evaluation, treatment, and discharge planning for patients referred to their services across acute care, skilled facilities, rehab and outpatient settings.

The role collaborates with other health professionals, mentors students, documents patient progress, and supports ongoing clinical operations while upholding the system's mission and values.

Qualifications

• PT degree with licensure in the state of practice.
• Experience in evaluating and treating patients across settings.
• CPR/BLS certification required.

Responsibilities

• Interacts with staff to provide evaluations, treatment, and discharge planning for a full patient caseload.
• Participates in student education (ICE, OT level I) or full-time/part-time options.
• Assesses literature and applies evidence to clinical practice.
• Plans, organizes, and delivers treatment programs to help patients regain function.
• Documents health progress, treatments, evaluations, and discharge summaries.
• Supports staff orientation, mentorship, and competency assessment.
• Implements program development or performance improvements with supervisor approval.
• Performs duties as assigned and supports departmental operations.
