Qualifications

  • Graduation from an accredited School of Pharmacy
  • State of Washington Pharmacist License
  • Two years of pharmacy practice experience

Responsibilities

  • Collaborate with providers to individualize drug therapies for safe, effective patient care
  • Perform comprehensive medication therapy review and monitor transitions of care
  • Monitor and modify drug regimens with required labs and assessments
  • Provide refill authorization with adherence and safety considerations
  • Serve as a drug information resource for providers, nursing, research staff and patients
  • Ensure access to specialty medications and documentation for insurers
  • Identify under-treated patients and optimize therapy
  • Educate patients on self-management of disease and drug therapy
  • Review prescribing patterns to educate on UWP guidelines
  • May manage anticoagulation, infectious disease, CAM therapies or pain care under a collaborative agreement
  • Dispense in various clinic and infusion settings
  • Provide mentoring to residents and students
  • Present at educational conferences and provide didactic teaching

Education

PharmD or equivalent
WA Pharmacist License
Pharmacy degree required
