Job summary

VITAS Healthcare Corporation in Lakeland, Florida is seeking a Nurse Practitioner to join the core interdisciplinary group providing consultation in palliative care, symptom management and supportive care for seriously ill patients and families.

You will collaborate with physicians and the palliative care team to implement care plans, educate staff, and participate in on-call rotations while maintaining patient records and complying with licensure requirements.

Qualifications

Master's of Nursing from an accredited university.
Licensed to practice in the state as a Nurse Practitioner (NP).
Certifications by a national body (ANCC/AANP); ACHPN preferred.

Responsibilities

Facilitate clarification of patient and family goals of care.
Facilitate access to hospice, rehabilitation, or ongoing aggressive treatment as appropriate.
Assist with diagnosis, treatment and management of acute and chronic conditions.
Educate facility staff on palliative care and services provided by VITAS Palliative Care.
Attend and lead team meetings and collaborate with physicians on care plans.
Prepare and maintain accurate patient records and documentation for care and reimbursement.
Prescribe medications, including controlled substances, after collaboration with physicians.
Order diagnostics and treatments as needed in collaboration with the care team.
Interpret diagnostic findings and communicate results to appropriate parties.
Collaborate with and supervise palliative care team members.
Contribute to development of clinical guidelines and standards.
