Job summary

Vista Community Clinic (VCC) in Vista, CA is hiring an experienced Registered Nurse to serve as a liaison between physicians, nurses, patients and other providers, handling medical condition questions and documenting calls.

Qualifications

• Graduate of accredited nursing program.
• Current licensure by the state of California (RN).
• Current CPR certification.
• Minimum five years' experience in acute care nursing and/or outpatient clinic; triage experience preferred.
• Broad knowledge base of general nursing practice; scope of practice appropriate to licensure.

Responsibilities

• Serve as liaison between physicians, nurses, patients and other providers to determine patient needs.
• Manage and document patient phone calls pertaining to medical conditions and questions according to established department routine.
• Assess urgency of patient problems and provide guidance on medications and health education.
• Process prescriptions and document all prescription/refill information.
• Provide nursing assessment support and triage per policy and SOAP notes.
