Job summary

EmpowerMe Wellness is seeking licensed therapists to deliver physical, occupational, or speech therapy to older adults within our EmpowerMe 360 program. You will assess residents, craft personalized care plans, and provide evidence-based interventions in a collaborative team environment.

As part of a comprehensive care model, you will support care coordination with wellness check-ins and telehealth support, while upholding professional standards and contributing to program development.

Qualifications

• Active license in therapy discipline (PT/OT/SLP)
• Experience with seniors preferred
• Passion for empowering seniors to live independently

Responsibilities

• Provide therapy and wellness services for older adults
• Conduct assessments, create care plans, deliver evidence-based interventions
• Support care coordination with wellness check-ins and telehealth
• Maintain professional and ethical standards
• Contribute to program development and quality improvement
