Job summary

Prairie Heights Healthcare in Aberdeen, SD seeks an Occupational Therapist to evaluate residents, determine needs, and develop individualized treatment plans to improve mobility, strength, balance, and independence, coordinating with physicians, nursing staff, and families.

You will supervise Certified Occupational Therapy Assistants, provide in-services, maintain compliant documentation, and participate in care conferences and quality improvement initiatives.

Qualifications

• Bachelor’s degree in Occupational Therapy; Master’s/Doctorate preferred.
• Licensed OT in the state; continuing education required.
• Proficient with rehab software (Net Health, Point Click Care) and documentation.

Responsibilities

• Evaluate residents to determine OT needs, functional limitations, and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility, strength, balance, endurance, and independence.
• Supervise Certified Occupational Therapy Assistants in direct resident care and related activities.
• Communicate progress with physicians, nursing staff, residents and families; participate in care conferences.
• Maintain timely, accurate, and compliant documentation; ensure infection control and safety protocols.
