Avir Health Group is seeking a Nurse Practitioner (Skilled Nursing) to lead clinical care in a Skilled Nursing Facility. You will work with the facility care team to oversee both short- and long-term residents, including admissions, discharges, and ongoing evaluations.

You will handle advanced care planning, comprehensive assessments, and prescribing treatments, while prioritizing patient safety and quality of life in a collaborative, supportive environment.

Qualifications

• Active Texas APN licensure and DEA license required.
• Master’s degree in Nursing.
• Experience in nursing/clinical settings with strong teamwork.

Responsibilities

• Perform monthly stability checks and evaluate changes in condition.
