Job summary

Santa Clara Valley Healthcare (SCVH) seeks qualified social workers to provide skilled services to patients, families, and staff across multiple hospital sites. The role requires a Master’s degree in Social Work and at least four years of progressively responsible experience, with opportunities across SCVH hospitals and clinics.

Qualifications

• Master's degree in Social Work from an accredited college.
• Four years of increasingly responsible social work experience, with at least two years at the level of Medical Social Worker I.

Responsibilities

• Provides social work services to patients/clients and families with complex bio-psycho-social adjustment issues.
• Develops intervention plans based on careful assessment of bio-psycho-social factors.
• Works with patient/client and family on implementing these plans with independence and initiative.
• Interprets bio-psycho-social, emotional and economic factors to health care staff.
• Prepares written social assessments and follow-up notes for patient records.
• Participates in staffing, interdisciplinary case conferences and presents relevant material.
• Performs related work as assigned.
