The Bradford at Brookside in Livingston, TX is seeking a dedicated LVN/LPN Charge Nurse for our Skilled Nursing team. This full-time daytime role focuses on delivering high-quality patient care within the LVN/LPN scope and guiding nursing support staff.

Qualifications

Graduation from an accredited Nursing School
Valid LVN/LPN license or License Practical Nurse license

Responsibilities

Participate in and oversee patient care within the scope of the LVN/LPN licensure.
Observes, assesses, and reports patient conditions/changes and documents in accordance with policies & regulations.
EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
Ensures implementation of care plans and administers medications and treatments as prescribed.
Participates in assigning duties and supervising the CNA staff to ensure patient well-being.
