Job summary

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for a per diem opportunity to serve adult and geriatric patients across Cuyahoga, Summit, and Medina counties. Prior home care experience is a plus but not required.

You will be part of a multidisciplinary team delivering rehabilitative care in patients' homes. Responsibilities include evaluating and treating speech, language, and swallowing issues, creating treatment plans, updating care plans, and educating

Qualifications

• Licensed Speech Language Pathologist in STATE (Ohio).
• Certificate of Clinical Competence in Speech from ASHA or equivalent qualifications.
• Experience meeting state licensure requirements and preparing for certification exams.
• Experience with EMR systems is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess and treat speech, language, communication, or swallowing disorders.
• Develop and adjust treatment plans based on evaluations and physician orders.
• Collaborate with BAYADA clinicians in multidisciplinary care plans.
• Educate families on rehabilitative care and activities for independent living.
• Document observations and interventions in a state-of-the-art EMR.
