Job summary

H2 Health Prime Living in Destin, FL is seeking a Physical Therapist to provide 1:1 care within a luxury Senior Living community. The role supports both Part-Time or Full-Time schedules, with a clinician-led, patient-focused culture.

We welcome new graduates and value a commitment to patient care, collaboration with a multidisciplinary team, and ongoing professional growth through in-house resources and mentorship.

Qualifications

• Must have degree from an accredited Physical Therapy or Physical Therapist Assistant program.
• Active or eligible state license to practice PT or PTA.
• New graduates welcome; outpatient experience a plus.

Responsibilities

• Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
• Design and implement personalized treatment plans to restore function and improve quality of life.
• Document patient progress accurately using EMR.
• Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
• Maintain a clean, professional clinic environment reflecting H2 Health’s culture of excellence.
