Job summary

Reliant Rehabilitation seeks a Physical Therapist to provide services in assisted living, independent living and memory care settings. You will follow a plan of care, supervise rehab techs as needed, and coordinate with a licensed PT to deliver quality patient care across community sites.

Maintain professional conduct, comply with state and federal regulations, and travel between communities as required, ensuring proper documentation and timely patient progress notes.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Maintain valid state license (where applicable).
• Willingness to travel between communities.

Responsibilities

• Under the direction of a licensed PT, optimize patients' functional abilities in the communities.
• Assist in initial evaluation and implement treatments for assigned patients.
• Ensure accurate and timely billing and documentation per guidelines.
• Maintain patient-centered interactions with the interdisciplinary team.
