EmpowerMe Wellness seeks a licensed therapy professional to deliver PT/OT/SLP services to seniors in Chicago under the EmpowerMe 360 model, integrating therapy with wellness and medical care.

You will assess residents, develop personalized care plans, and collaborate with a multidisciplinary team to help them stay independent, safe, and engaged in daily living. Flexible schedule and community presence are emphasized.

Qualifications

• Active professional license in therapy disciplines (PT/OT/SLP).

Responsibilities

• Provide therapy and wellness services (physical, occupational, or speech) for older adults
• Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team
• Support care coordination with wellness check-ins, caregiver communication, and telehealth support
• Maintain professional and ethical standards in line with company initiatives
• Contribute to program development and continuous quality improvement
• Help residents maximize independence, confidence, and satisfaction
