Job summary

Americare Senior Living - Sabetha Manor seeks a detail-oriented MDS Coordinator to support accurate assessments, regulatory compliance, and high-quality resident outcomes within our skilled nursing community. The role coordinates MDS processes, ensures timely documentation, and collaborates with nursing leadership and the interdisciplinary team to drive care planning and Medicare workflows.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license required.
• Experience with MDS 3.0 in a skilled nursing setting.
• Strong attention to detail, organization, and analytical skills.
• Ability to collaborate effectively with interdisciplinary teams.

Responsibilities

• Coordinate and complete MDS assessments per federal and state guidelines.
• Ensure admission nursing notes, care plans, and clinical documentation are accurate and current.
• Complete assigned MDS sections, assessments, and RAP summaries.
• Review, update, and maintain resident care plans based on assessments.
• Participate in Medicare meetings, care plan meetings, and interdisciplinary team discussions.
• Maintain compliance with state, federal, and Americare standards.
• Support survey readiness and participate in inspections as needed.
• Communicate changes in resident condition to leadership.
• Assess nursing supply and equipment needs.
• Provide on-call support to staff as needed.
