Job summary

Cogir Senior Living seeks a Health and Wellness Director (LPN/LVN or RN) to lead clinical operations in a memory care community. The role partners with the Executive Director to ensure high-quality resident care and regulatory compliance.

You will oversee assessments, care plans, medication programs, staff hiring/supervision, and collaboration with families. Strong organizational and computer skills are essential.

Qualifications

• RN/LVN or RN license in good standing.
• Associate degree or higher in nursing; management coursework is a plus.
• Current state-required certifications (First Aid, CPR, etc.). Proficient in computer literacy and Microsoft Office.

Responsibilities

• Foster a resident-centered approach and respect residents' rights.
• Assess health, functional, and psychosocial status; initiate care plans.
• Establish and maintain the medication program.
