Mosaicms in Oregon seeks a Community Relationship Manager to guide families to a vibrant senior living community and drive outreach through medical offices, centers, and referrals. You will present services and coordinate tours, building lasting relationships with discharge planners and clinicians.

The role emphasizes a relationship-driven sales approach, adherence to the established process, and accurate record-keeping to support admissions and community growth.

Qualifications

• Medicaid payor source approval in writing from a caseworker prior to admission.
• Collaboration with clinical team to ensure physician orders and evaluations are completed before admissions.
• Coordinate with business office to set up contract signing prior to admission.
• For Memory Care Communities ensure diagnosis of Dementia and assign POA/Guardian prior to admission or evaluation.

Responsibilities

• Provide the highest level of customer service by building inquiry base and initiating marketing calls to discharge planners, social workers, and other community professionals.
• Visit and present at doctors' offices, senior centers, clinics, and organizations for seniors distributing sales literature.
• Conduct home visits, giving an overview of services, lifestyle, and community benefits.
• Follow the company’s sales process, guidelines, and philosophy.
• Complete database documentation and keep executive director and department heads updated on outreach plans and events.
• Represent the community to welcome potential residents, families, referral sources, and the public with a positive, helpful presence.
• Maintain accurate database records in Bluestep for professional referral sources.
• Prepare and distribute marketing reports timely and accurately as needed.
• Perform competitive analysis every six months or as requested.
