Centers Health Care is seeking a Recreation Aide for the Boro Park Center for Rehabilitation and Healthcare in Brooklyn, NY. The ideal candidate will plan, develop and lead engaging recreation programs for residents, supporting their well-being and enjoyment.

Responsibilities include creating monthly activity calendars, documenting participation in SigmaCare or PointClickCare, collaborating with the care team, and assisting residents on program days.

Qualifications

• High school diploma or equivalent.
• Proficiency with electronic medical record systems (SigmaCare or PointClickCare).
• Ability to work as part of a team and multitask.
• Experience with recreation therapy/activities is a plus.
• Knowledge of person-centered care philosophy.
• Excellent communication and interpersonal skills.

Responsabilités

• Plan, develop, and lead engaging recreation programs tailored to residents' interests, abilities, and needs.
• Create and maintain monthly activity calendars.
• Document participation and progress using SigmaCare, PointClickCare, or similar documentation systems
• Collaborate with interdisciplinary team members to support resident well-being and individualized care plans
• Ensure compliance with regulatory standards while promoting a positive, inclusive environment
• Versatile in organizing supplies and assisting with program setup
