  • Provide staff training in Ryan White Case Management and other subprogram areas.
  • Submit programmatic reports monthly to program manager/coordinator.

This position serves as Ryan White Case Manager and is responsible for the intake/re-determination, financial assessment, updating demographic information in the HRSA CAREWare computer program, issuing URN cards, eligibility acuity, obtaining consents, rights & responsibilities and the Share of Cost assessment. All documentation regarding the client eligibility status will be placed in CAREWare.

The Ryan White Case Manager will maintain confidentiality of client information at all times. Respects the confidentiality of related issues by not discussing any information with unauthorized staff. Ensure discussions held between authorized staff cannot be heard by unauthorized staff. Reports breaches of security to supervisor immediately. Specific duties are as follows: Conducts face-to-face interviews with clients to determine eligibility for clients that may be eligible for Ryan White core services using state guidelines (Florida HIV/AIDS Eligibility Procedures Manual). Maintains accurate and current client data and information using State approved programs including CAREWare, HMS, ADAP (Provide), FLMMIS, WAGES, Change Healthcare, and other programs as assigned. Creates client charts composed of current forms, signed releases, progress notes, medical data and demographic information for all enrolled clients. Completes monthly, quarterly and annual reports to supervisor. Coordinates care of clients with DOH-Volusia clinical team. Assists with implementation of health literacy education service program to identified clients.

Maintains current HOPWA program knowledge and/or seeks guidance to adhere to state and federal regulations. Ensures knowledge of benefits of agency and local partners that may be applicable to Ryan White clients and provides referrals as needed. Provides HIV education regarding the importance of medical adherence and retention in care, transmission of HIV to sex and needle-sharing partners. and making healthy lifestyle choices. Participates in community events as requested. Provides staff training in Ryan White Case Management, ADAP, Eligibility and other subprogram areas applicable to role. ADAP program - Provides social services to individuals or families in need of AIDS Drug Assistance Program (ADAP) services. Conducts face-to-face interviews, enters all pertinent client information into ADAP database (Provide), enrolls clients in Pharmaceutical

Assistance programs as needed, assists clients with required paperwork and provides education and referral services to clients. Participates in quarterly conference calls as requested by the Department. Submit monthly programmatic reports to the program manager/coordinator by the tenth day following the end of each month.

Miscellaneous Activities
  • Attends and participates in general staff meetings, staff development, such as team/staff meetings and in-service training/continuing education.
  • Attends computer skills training classes as needed.
  • Maintains timely submission of EARS, timesheets, telephone invoices, and travel documentation.
  • Performs other duties as assigned.
