PromptCare in Plainview, NY is seeking experienced Registered Home Infusion Nurses to provide specialized care in patients' homes. You will assess, plan, implement, and evaluate nursing care, administer IV medications, and educate patients and families about home care, medications, and treatments.

The role requires RN licensure, CPR, and at least 2 years of home infusion experience; on-call availability, valid driver's license, and knowledge of IV therapies are essential.

Qualifications

• Must be a Registered Nurse in the state of employment
• Licensed driver and automobile insurance
• CPR certification required
• Knowledge of IV therapies and vascular access devices
• At least 2 years’ home infusion nursing experience or equivalent
• ED experience preferred
• Heart failure knowledge or certification desirable
• Pediatric IV skills preferred
• CRNI certification preferred
• Experience with inotropes, IVIG, and TPN is a major advantage

Responsabilités

• Assess, plan, implement, and evaluate patient care plans in home infusion settings
• Educate patients and families about home care, medications, and treatments
• Conduct initial nursing assessments for home infusion therapy
• Train patients/caregivers on procedures and equipment
• Develop and revise care plans with the healthcare team
• Visit patients, document compliance, and provide direct care
• Manage on-call duties and patient supplies
• Coordinate care with other agencies and counsel patients on therapy
• Collaborate with Pharmacists for therapy adjustments
• Plan efficient daily routes and submit nursing notes promptly
• Triages field problems and adhere to SOPs and regulations
• Deliver high-quality service and assist with travel expense reports
• Provide weekend and evening nursing services as required
• Perform other duties as assigned
