Job summary

CenterWell Home Health in Bluffton, SC is seeking a Clinical Manager to lead a team of clinicians delivering in-home care. You will guide clinical practice, assure documentation quality, and support staff through coaching, performance reviews and training.

The role requires an RN with at least 2 years of home health experience, CPR certification, and a valid driver's license. OASIS experience and Home Health Homebase knowledge are preferred.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state license as a Registered Nurse.
• Proof of current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• A minimum of 2 years of experience as a Registered Nurse.
• Home health experience or related home-based care background preferred.
• Management and people leadership experience required.
• OASIS experience preferred; certifications (HCS-O/COQS/COS-C) preferred.
• HCHB experience preferred.
• Foundational CMS PDGM knowledge preferred.

Responsibilities

• Guide, support, and educate clinicians; assist with care planning and clinical decision making.
• Ensure clinical documentation, audits, and billing meet Medicare and payer standards; monitor outcomes.
• Participate in hiring, training, performance evaluation, coaching, and corrective action.
• Review and audit clinical documentation (OASIS, visit notes, plan of care).
• Collaborate with field clinicians to ensure timely submission of documentation and resolve deficiencies.
• Assist with OASIS accuracy and validation; ensure data entry aligns with care plans and physician orders.
• Monitor quality indicators and assist in performance improvement plans.
• Participate in quality improvement projects and regulatory readiness.
• Track and report quality metrics to leadership.
• Provide staff education and support on compliance and documentation standards.
• Stay current on federal/state home health regulations.
