Job summary

Tufts Medicine is seeking an Advanced Heart Failure Nurse Practitioner/Physician Assistant to join our team in Massachusetts. The role focuses on comprehensive evaluations, order and interpret diagnostics, and collaborate with physicians to diagnose and treat acute and chronic conditions.

The incumbent will develop individualized care plans, educate patients and families, and participate in rounds with residents and the medical team.

Qualifications

• Bachelor’s degree.
• PA program completion.
• PA licensure within 3 months of complete medical staff application.
• PA certification.
• BLS certification.

Responsibilities

• Manage a broad spectrum of patients and perform clinical, diagnostic, and therapeutic procedures.
• Assess health status through histories, exams, and labs; diagnose and treat acute and chronic conditions.
• Coordinate care plans with the healthcare team and supervise patient discharge when appropriate.
• Order labs and imaging; interpret results and adjust treatment plans with supervising physician.
• Maintain accurate records and document care in the medical record system.
