Job summary

Salgado-LLC is seeking a dedicated Physical Therapist to join a value-based, mission-driven team. This role emphasizes building long-term relationships with a smaller panel of seniors, delivering restorative care instead of rapid-volume treatment, and integrating with an interdisciplinary clinical team.

You will design and supervise personalized rehabilitation plans, oversee PTAs, and ensure patient safety and independence at home or in the facility.

Qualifications

  • Requires California Physical Therapy License.
  • Bachelor’s degree in Physical Therapy from an accredited program.
  • CPR and First Aid certifications within 180 days of hire.

Responsibilities

  • Holistic Assessments: Conduct thorough PT assessments in facility and occasionally in patients’ homes.
  • Therapeutic Interventions: Deliver active and passive exercise, muscle re-education, gait training, ADL training.
  • Advanced Modalities: Apply modalities like massage, heat, sound, water, light, or electrical.
  • Team Care Planning: Participate in team meetings to establish benchmarks and personalized care plans.
  • Supervision & Mentorship: Supervise PTAs to implement Functional Maintenance Programs.
  • Equipment & Safety Training: Train staff, patients, and caregivers on equipment safety.
  • Compliant Documentation: Document interventions in real time per guidelines.
