Qualifications

• Bachelor degree required.
• 3–4 years of broad clinical experience.
• 3–4 years in home care, discharge planning, or case management.
• 2 years in managed care preferred.

Responsibilities

• Assess and coordinate services to ensure appropriate care.
• Collaborate with families, physicians, and care teams.
• Organize care plans with clinical guidelines.
• Advocate for members across sites to optimize resources.
• Document care accurately per standards.
• Travel locally for field-based case management.
