Qualifications

• 1 year PGY-1 Residency (Required).
• 1 year PGY-2 Residency (Preferred).
• Licensed Pharmacist in Virginia within 60 days (Required).
• Board of Pharmacy Specialties within 2 years (Required).

Responsibilities

• Provides enterprise leadership of the formulary and systemwide governance, co-chairing committees and directing implementation.
• Leads utilization management strategies to optimize drug-use criteria and step-therapy protocols.
• Oversees high-cost therapies, formulary monograph development, and pharmacoeconomic analyses.
• Directs standardization of medication products, concentrations, and therapeutic interchange programs.
• Provides budget impact and total-cost-of-care analyses to guide formulary decisions.
