Job summary

Avir at Abilene in Abilene, Texas seeks a Licensed Social Services Worker to assess residents' social and emotional needs and provide medically related social services to support physical, mental, and psycho-social well-being.

Responsibilities include admissions tasks, resident transfers, acting as Ombudsman, committee participation, care plan development, and coordinating outpatient appointments. Requires MSW, geriatric experience, and active licensure.

Qualifications

• Master’s degree in social work is required.
• Two years of experience working in geriatrics.
• Valid Driver’s license is required.
• Must have state licensure.
• Previous experience with nursing homes preferred.

Responsibilities

• Ensures the accuracy and completion of resident admission tasks including correspondence and facility tours.
• Oversees the transfer of residents to/from home or to another facility.
• Serves as the Ombudsman for the residents.
• Attends resident council meetings, care plan meetings, and other appropriate committee meetings.
• Ensures social service assessments are completed for each resident resulting in Individualized Care Plan.
• Monitors and controls social service progress notes
• Coordinates the assignment of outpatient appointments, including transportation.
• Ensures residents' personal needs are met
• Manages social services assistants to ensure accuracy and completion of assigned tasks
