Job summary

Jobtailor is seeking an experienced regional Clinical Leader to drive multi-state clinical governance and operational excellence in home healthcare. You will coach regional DONs/Administrators, align quality metrics with executive targets, and ensure readiness for CMS, ACHC, and state surveys.

The role requires a BSN, active RN license, multistate eligibility, and 5+ years of progressive leadership in multi-site home health.

Qualifications

  • BSN is required and RN license must be active.
  • Multistate/Compact license eligibility required with willingness to obtain licensure in additional states within the region.
  • 5+ years of progressive home healthcare experience.
  • 5+ years of clinical leadership experience in a home health or multi-site setting.
  • Demonstrated experience managing multi-site teams and leading agencies through regulatory surveys.

Responsibilities

  • Provide strategic, multi-state clinical leadership and operational governance across a designated region.
  • Directly manage, coach, and hold local Directors of Nursing/Administrators accountable.
  • Aggregate, analyze, and present regional clinical and quality performance metrics to the executive team.
  • Translate corporate clinical strategies into standardized operational practices across multiple markets and states.
  • Partner with Regional Senior General Managers to align clinical quality with market growth targets.
  • Collaborate with HR, Compliance, Finance, and Product/IT teams to optimize operations.
  • Oversee regional clinical capacity models for census growth, service line expansions, and geographic surges.
  • Standardize onboarding, mentoring, and professional growth pathways for local DONs/Administrators.
  • Monitor nursing turnover and burnout trends and implement clinician well-being and engagement strategies.
  • Drive regional performance against clinical quality strategy, QA/QAPI initiatives, and utilization benchmarks.
  • Review and elevate complex clinical edge cases, high-risk patient events, and severe adverse incidents.
  • Lead regional root‑cause analyses and implement systemic corrective adjustments.
  • Standardize continuous survey readiness for CMS Conditions of Participation, ACHC/Joint Commission accreditation, and state-specific regulations.
  • Oversee preparation for and response to state surveys and federal audits.
  • Review and sign off on local Plans of Correction before execution.
  • Identify, track, and mitigate legal, financial, and clinical operational risks.
  • Partner with Product/IT to reduce clinician documentation burdens and standardize clinical workflows.
  • Minimize unwarranted practice variations while ensuring adherence to state nursing practice acts.

Requirements
  • Bachelor of Science in Nursing (BSN) required
  • Current, unrestricted Registered Nurse (RN) license
  • Multistate/Compact license eligibility required
  • Willingness to obtain licensure in additional states within the region as necessary
  • Minimum of 5+ years of progressive home healthcare experience
  • 5+ years of clinical leadership experience in a home health or comparable multi‑site setting
  • Demonstrated experience managing multi‑site teams
  • Experience leading agencies through state/federal regulatory surveys
  • Experience scaling clinical operations during rapid growth
  • Successful completion of a background check
  • Satisfactory references
