Job summary

Hackensack Meridian Health is seeking a Senior Community Outreach Coordinator to lead the planning, coordination, and execution of the organization’s Community Health Needs Assessment (CHNA). The role develops strategies to address identified community health needs in alignment with HMH's strategic imperatives.

The ideal candidate has an Associate's degree, 5+ years coordinating outreach in non-profit settings, and strong communication and computer skills.

Qualifications

• Associate's degree required.
• Excellent organization and communication skills.
• 5+ years experience coordinating outreach programs in the non-profit community setting (social services, public health, healthcare, education) with diverse populations (children, seniors, etc).
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.

The Senior Community Outreach Coordinator is responsible for planning, coordinating and implementing Hackensack Meridian Health’s (HMH) Community Health Needs Assessment (CHNA) as well as developing strategies to address identified community health needs and in support of Hackensack Meridian Health’s strategic imperatives and centers of excellence.

Education, Knowledge, Skills and Abilities Required:

• Associate's degree.
• Excellent organization and communication skills.
• 5+ years experience coordinating outreach programs in the non-profit community setting (social services, public health, healthcare, education) with diverse populations (children, seniors, etc).
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms.

• Bachelor's or Master's in public health or related field.
• Bi-lingual (Spanish).

Licenses and Certifications Required:

• Current Driver's License from a USA state.

Licenses and Certifications Preferred:

• Certified Health Education Specialist (CHES).
