Job summary

The Licensed Mental Health Professional will join a NY OMH-funded Adult Home based care transition team, serving individuals moving from Adult Home to the community. You will perform discharge planning, group and individual counseling, and develop daily living skills and coping strategies.

Engage with community providers, conduct psychosocial assessments, coordinate housing supports, and participate in on-call coverage to support smooth transitions and ongoing community linkage.

Qualifications

• Master’s degree or higher in social work, mental health counseling, nursing or psychology.
• 2+ years post-masters work experience in mental health/criminal justice preferred.

Responsibilities

• Engage at Adult Home for transition planning and needs assessment.
• Monitor discharge planning pre- and post-transition.
• Collaborate with DOH, Managed Care Plans, and Housing Contractors.
• Provide short-term counseling and risk assessment/safety planning.
