Qualifications

Doctor of Pharmacy (PharmD) or Bachelor's Degree in pharmacy.
5 years of experience in the advanced practice area; or 3 years PGY1 residency and 2 years in advanced practice; or 2 years PGY1 and PGY2.
AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) required.
California Pharmacist License required.
Pharmacy-Based Immunization Delivery certificate required.
Board of Pharmaceutical Specialties (BPS) certification required within 6 months of hire.
Advance Practice Pharmacist License (CA Board of Pharmacy) required within 12 months of hire.

Responsibilities

Review prescriptions for appropriate dose, duration, frequency, and indication; monitor for drug interactions.
Provide patient education tailored to the regimen and conduct medication reconciliation.
Participate in Clinical Services including CDTM and treatment team activities; document outcomes.
Collaborate with health care team to review formulary status, guidelines, and care maps.
Counsel patients on medication use and safety; ensure accurate documentation.
