About ThedaCare! Summary

The Clinical Dietitian, Senior assesses nutritional status and develops and implements a comprehensive nutrition care plan. Completes nutrition focused physical assessments to accurately diagnose malnutrition. Supports writing and ordering parenteral and enteral nutrition for all hospitalized patients seven days per week. Participates in the interdisciplinary team providing care coordination, education, and support during the discharge process. Assists with planning, development, and implementation of department programs, policies, and goals.

Job Description

1. Develops, implements, monitors, and coordinates nutritional care plan based on assessment to client's needs with other health professionals.
2. Writes orders for enteral and parenteral nutrition, supplements, and additives as indicated by the assessment and plan of care.
3. Works seven days per week including holidays to support ThedaCare's hospitalized patients on nutrition support (enteral and parenteral nutrition).
4. Responds to consults within 24 hours, including patients in ambulatory settings identified with nutrition risk per nutrition risk screen.
5. Completes nutrition focused physical exam to diagnose malnutrition.
6. Counsels individuals and family members on nutritional principles, dietary plans, food selection, and economics of nutrition.
7. Adapts plans to individual client's needs, lifestyle, and cultural eating habits.
8. Provides follow-up and makes appropriate referrals based on needs of individual.
9. In collaboration with the dining and patient meal department, plans and evaluates menus to meet nutritional needs of patients with consideration for food preferences.
10. Documents assessments, recommendations, and plan of care in EPIC.
11. Documents in EPIC care plans and education interventions and goals.
12. Participates in discharge planning.
13. Recommends home care nutritional needs.
14. Interprets, evaluates, and utilizes current research to achieve effective nutritional care.
15. Utilizes the Nutrition Care Manuel from the Academy of Nutrition and Dietetics as a resource for educational materials.
16. Participates in process improvement work to support delivery of best care to patient in the acute care setting.
17. Oversees dietetic technician assessments and interventions per defined scope of practice per the Academy of Nutrition and Dietetics.
18. Serves as a preceptor for dietetic interns.

Qualifications

• Registered Dietitian
• One year of experience in hospital nutrition
• Certified Dietitian

Physical Demands

• Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
• Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

• Climate controlled office setting with daily movement throughout the facility
